YATDS Project


The Young Adult Transition to Day Services (YATDS) team is a new service that aims to support young people with complex transition needs who are currently leaving school or a rehabilitative training programme and transitioning to a HSE-funded disability day service. This is a cross-agency initiative involving the Brothers of Charity Services Ireland – West Region working with Mayo HSE Community Services, linked closely with the HSE Day Opportunities Office in CHO2.


Clinical supports from the team can be provided in various ways: Universal Supports, Targeted Supports and/or Individualised Supports. 

Support staff teams to:

  • Enrich the young adult’s environment,
  • Visually support the young adult’s environment
  • Support the young adult to participate in their day service to their full potential

Staff training focuses on strategies to support the young adult to achieve their personal goals.

Small group support with the young adult and their peers focuses on a particular challenge best met in a group setting.

A more intensive level of support is delivered to the young adult in a 1:1 format.

Once a goal is met, and/or the clinical supports needed to meet this goal are in place, then this episode of care by the team is finished. Supports are based on the strengths and needs of the individual young adult and the resources available to the YATDS at that time.
